This Privacy Policy describes how Jack Thomas, an Arizona-licensed insurance producer doing business as Jack Thomas Health Insurance ("Jack Thomas," "I," "me," or "my"), collects, uses, and discloses information about you when you visit this website or interact with services offered through it.
I take privacy seriously. The volume of information I collect is small, and the uses are limited to what's necessary to operate this business. This policy explains what's collected, why, and what your rights are.
Who I am
Jack Thomas is a sole-proprietor insurance producer licensed in the State of Arizona (License #22137766) and certified by the Federal Health Insurance Marketplace (FFM). I am appointed with insurance carriers serving self-employed Arizonans. Nothing on this website is offered by, endorsed by, or affiliated with the U.S. Government, HealthCare.gov, the Health Insurance Marketplace, or the Medicare program.

When you book a consultation through the calendar tool on this site, you provide certain information directly, including:

- Your email address
- Your phone number
- Any optional notes you choose to include about your situation
- The date and time you select for the call
If we speak by phone or email after that initial booking, you may share additional information relevant to evaluating insurance options, such as your age, household size, household income, current coverage, or health-related information necessary for plan selection.

Calendly
Consultation bookings are processed through Calendly, a third-party scheduling service. When you book a call, the information you submit (name, email, phone, scheduling details) is collected and stored by Calendly on my behalf. Calendly's own privacy practices apply to that data; their privacy policy is available at calendly.com/privacy.
Meta (Facebook) Pixel
This website uses the Meta Pixel, a tracking technology provided by Meta Platforms, Inc. The pixel collects information about your visit (including IP address, pages viewed, and certain actions taken on the site) and shares that information with Meta. Meta uses this information to measure the effectiveness of advertising, deliver targeted advertising, and build audiences for advertising campaigns. How I share your information
I do not sell your personal information for money. I share information only in the following circumstances:
- With insurance carriers, when you choose to enroll in a plan or apply for coverage. The information shared is what's necessary to complete the application.
- With service providers that help me operate this business, such as Calendly (scheduling), my email provider, and my web hosting provider. These providers process information on my behalf and are not permitted to use it for their own purposes.
- With Meta and similar advertising platforms, as described above. As noted, this may be considered a "sale" or "sharing" under certain state privacy laws.
- To comply with law, including responses to subpoenas, court orders, regulatory inquiries from state insurance departments, or other lawful requests.
- To protect rights and safety, including investigating potential fraud, enforcing my agreements, or protecting myself or others from harm.
Insurance information and confidentiality
As a licensed insurance producer, I am subject to the privacy provisions of the Gramm-Leach-Bliley Act (GLBA) and applicable Arizona insurance privacy regulations. Nonpublic personal information you share with me in the course of an insurance consultation, application, or enrollment is treated as confidential and is used only for the purposes for which you provided it. It is shared with carriers and authorized third parties only as necessary to provide the services you've requested or as required by law.
Your rights
Rights under California, Colorado, Virginia, Connecticut, and similar state laws
Depending on your state of residence, you may have one or more of the following rights regarding your personal information:
- Right to know / access: You may request a copy of the personal information I have about you.
- Right to correct: You may request correction of inaccurate personal information.
- Right to delete: You may request deletion of personal information, subject to legal exceptions (such as records I am required to retain under insurance laws).
- Right to opt out of sale or sharing: You may opt out of the sharing of your personal information with advertising platforms such as Meta. To opt out, email me at the address below. You can also limit Meta's use of your data through Meta's own controls in your Facebook or Instagram account settings.
- Right to non-discrimination: I will not deny you services, charge you a different price, or provide a different level of service because you exercised any of these rights.

You may designate an authorized agent to make a request on your behalf. I will require written verification of the authorization before processing the request.

I retain personal information for as long as necessary to provide the services you've requested, comply with legal and regulatory obligations (including insurance recordkeeping requirements that can extend several years after a policy ends), resolve disputes, and enforce agreements. When information is no longer needed, it is deleted or de-identified.
Security
I take reasonable measures to protect personal information from unauthorized access, alteration, disclosure, or destruction. No method of transmission over the internet or electronic storage is completely secure, and I cannot guarantee absolute security. If you believe your information has been compromised, please contact me immediately.
Children
This website is not directed to children under 16, and I do not knowingly collect personal information from children under 16. If you believe a child has provided personal information through this site, please contact me and I will delete it.
Changes to this policy
I may update this policy from time to time to reflect changes in my practices, the services I use, or applicable law.
