This website is operated by Jack Thomas, an Arizona-licensed insurance producer (License #22137766) doing business as Jack Thomas Health Insurance. The site is an advertisement for insurance services and a means to schedule a consultation.

Nothing on this site constitutes an offer to sell insurance, a binding insurance quote, or a contract. Insurance coverage can only be obtained through a formal application process with a licensed carrier and is subject to the carrier's underwriting and approval.

Not affiliated with the government

This site is not connected with, endorsed by, or affiliated with the U.S. Government, the Health Insurance Marketplace, HealthCare.gov, the Centers for Medicare and Medicaid Services, or the Medicare program. It is a private commercial advertisement for the services of an independent licensed insurance producer.

Eligibility

This site is intended for residents of Arizona who are at least 18 years of age. Insurance products discussed through this site are limited to those for which I am licensed and appointed in Arizona. Eligibility for any specific insurance product depends on the carrier's underwriting criteria, your age, your residence, your health history, and other factors.

Scheduling consultations

The calendar tool on this site allows you to schedule a consultation. By booking a call:

The consultation itself is provided at no cost. You are under no obligation to purchase any insurance product. If you choose to enroll in a plan, the carrier pays a commission to me; you do not pay an additional fee for my services.

Communications and consent

By providing your phone number through the booking tool, you expressly consent to receive calls and text messages from Jack Thomas related to your scheduled consultation and any insurance services you request. This consent includes calls and texts placed using automated systems. You may revoke consent at any time by replying STOP to any text message, by stating your withdrawal of consent on a phone call, or by emailing me at the address in the Privacy Policy.

Carrier message and data rates may apply to text messages. Frequency of messages depends on your interactions with me.

All content on this website, including text, graphics, logos, the wordmark "Jack Thomas Health Insurance," images, and the underlying code, is the property of Jack Thomas or used with permission, and is protected by copyright and other intellectual property laws. To the fullest extent permitted by Arizona law, in no event will Jack Thomas be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of this site, including but not limited to lost profits, lost data, or damages resulting from inability to use the site. My total liability for any claim arising out of or relating to this site is limited to one hundred dollars ($100).

Governing law and disputes

These Terms of Use are governed by the laws of the State of Arizona, without regard to its conflict-of-law principles. Any dispute arising out of or related to these terms or your use of this site shall be brought exclusively in the state or federal courts located in Maricopa County, Arizona, and you consent to the personal jurisdiction of those courts.

Regulatory complaints

If you have a complaint about my conduct as a licensed insurance producer, you have the right to file a complaint with the Arizona Department of Insurance and Financial Institutions. Information on filing a complaint is available at difi.az.gov.

Changes to these terms

I may update these Terms of Use from time to time. When changes are made, the effective date above will be updated. Continued use of the site after a change indicates acceptance of the updated terms.
